Payment Options

First and foremost, at Dr. Grubisa’s office our mission is to provide you with high-quality treatment in a comfortable, compassionate environment. We offer a variety of payment options to meet your financial needs. We have several different payment schedules, as well as payment methods (credit card, automatic debit) to meet your needs. Please speak with us and we will do our best to make your financial contract meet your specific financial requirements. We thoroughly explain all available options, and please don’t be shy to ask for more clarification if needed.

Insurance

Our financial coordinator will provide you with a completed Canadian Association of Orthodontists Insurance Predetermination Form prior to treatment commencement. You must submit this form to your insurance carrier(s) to determine if a portion of your treatment is covered by your insurance company.

Our office does not bill, nor receive payments from the insurance company directly. All contracts and charges are made, and payments received, directly from the patient or parent (responsible party). Payments receipts provided so that you can submit them to the insurance company for reimbursement by you, the policyholder.

An initial fee is due at the start of orthodontic treatment; however the balance can be paid over time on a payment plan, which is set up for automatic payments/withdrawals. These auto-payments can be set up with Visa, MasterCard or Debit card and for your convenience.
